How it works

  1.  Fill the intake testing form
  2.  Book a COVID 19 appointment and pay online by a credit card.
  3. When you arrive at Welltopia park on lot, PARK BY THE RIVER, remain in your vehicle and call us at : 262-429-9429. A pharmacy team member will come to supervise the test sample collection.
